​2020 Tribal Community Prevention Assessment
Purpose and Benefits
The goal of the 2020 Tribal Community Prevention Assessment is to collect Tribal specific data to identify health behaviors and practices.  Data will be used to select interventions that the Tribe will implement to improve the health of the community.  The 2020 Tribal Community Prevention Assessment is being conducted by your local Tribal Nation’s Minobimaadiziiwin Coalition, in conjunction with Great Lakes Inter-Tribal Epidemiology Center and BEAR Consulting, LLC, as part of Great Lakes Inter-Tribal Council’s Strategic Prevention Framework Partnerships for Success Grant (SPF PFS).    
Confidentiality
Your assessment is confidential.  Therefore, your name will not be connected to any of the information you provide; your responses will be combined with others.  We will not use your name when we report results.  To ensure your information is kept confidential, the survey proctors have completed training on data confidentiality.   They will keep all your information confidential.  All data will be secured and password-protected.
